The following donor levels will be listed in our 2010 Annual Report:

Walkers                                         all contributions up to $1,000

Joggers                                         from $1,000 to $4,999

Swimmers *                                  $5,000 or more

Cyclists *                                        $10,000 or more

Runners *                                      $25,000 or more

Coaches*                                       $50,000 or more

* Additional recognition & benefits provided to donors in these higher categories.

The Campaign for Public Health Foundation has been granted public charity status in accordance with Section 501(c)(3) of the U.S. Internal Revenue Code. All Contributions are Tax -deductible to the fullest extent allowed by U.S. law.
