Become a Member

The CPH Foundation is a member-based organization.  Members of the CPH Foundation can be individuals, organizations or businesses who contribute funds, provide pro-bono services or volunteer substantial effort to the organization.

Under our bylaws, the CPH Foundation does not offer members voting rights. We will list all members on the CPH Foundation’s web site and may print a list of members on the organization’s letterhead.  Additionally, all members will be included in the Foundation’s database and will receive the Foundation’s publications. Significant contributions will also be recognized in the Foundation’s Annual Report.

Walkers                                        all contributions up to $1,000

Joggers                                        from $1,000 to $4,999

Swimmers *                                $5,000 or more

Cyclists *                                      $10,000 or more

Runners *                                    $25,000 or more

Coaches*                                     $50,000 or more

The Campaign for Public Health Foundation has been granted public charity status in accordance with Section 501(c)(3) of the U.S. Internal Revenue Code. All Contributions are Tax -deductible to the fullest extent allowed by U.S. law.
